In order to secure your fundraiser idea, please review the procedures and fill out the request. Select your desired date based on the attached calendar and submit for approval. Once approved, Debra Mitchell or Janet Morris will add the fundraiser to the calendar and contact you to discuss the details. It is best to schedule your event at least 2 weeks from the date you submit your request. Fundraising Procedures click here. Fundraising Requests click here. You can fill the form out in Adobe Reader. When complete, "save as" to your computer. Send an email to: fundraiser@lcoos.org and attach your completed request.
